I have 5/5 in Mind Sear, and took the Mind Sear reward from and took from an escort Improve Mind Sear, but it did not improve .
The option should be disabled in this situation.

Uhm... It is? I hit 5/5 psiblades, and picked up a betrayal on an alchemist. After successfully escorting, the only reward choice was thorn grab. Are you sure you were all the way at 5/5 talent points, and not just an effective level of 5?

Yes. I had 6/5, 6.5 effective before escort.
