[RC2] Missing a Guardian in Ruined Dungeon

No Guardian spawned in the northwest most chamber, leaving the fifth orb from the left inactive.

Additional details:
This happened once before with RC1, so I was counting Guardians carefully as I killed them in Ruined Dungeon. Dug around the chamber and used illumination: Sun but did not find anything. Playing a Cornac Bulwark at lvl 21 when I went in, leveled to 22 while inside. Already unlocked Infinite Dungeon prior to this and got lore from Sher'Tul Fortress library, so did not pick up the first inscription - walked over the rest.

Also just happened to me in v1.0. 5/6 rares were generated, OR, I killed 6 but 5/6 triggered the orb events.
