[b37] Lithfengel doesn't show up (another Fearscape bug?)

I got the quest from Tannen to go to Reknor to get a new Athame and Diamond. Upon reaching Reknor (4), I didn't receive the usual message about orc corpses or whatever, and the demon that normally has the Athame and Diamond didn't spawn. Tried leaving and re-entering, talking to Tannen again, etc, but no luck. I suspect this might have something to do with the fact that I killed pretty much every boss inside the Fearscape, with Golbug the Destroyer being no exception.

I got around the issue with cheat mode, but this seems to be a pretty serious issue as it has the potential to be a game-ender right here, with no way of returning to the West if you gave Tannen the Orb.

I once had it spawn in the hidden treasure room...you're sure this wasn't the case?

Even if it did spawn in the hidden treasure room (I didn't know there was one, actually, thanks for the heads up) wouldn't it have still given that message upon entry that goes something like "the orc corpses you left behind are all gone now"?

Same issue, also a corrupter that (probably) fearscaped golbug (I say that as I fearscape most bosses, but don't remember him in particular). I always hold onto my orb, so I guess this character will just do a bit of extra walking and miss a dungeon.

The backup guardian in dreadfell appears to have not spawned, unless he was moved from the first floor, but that may be unrelated (I also fearscaped the master - that one I'm sure of).
