I accepted the quest and was teleported to the island. The level loaded enough for me to see the cultists around me trigger their particle effects. I then crashed, with the autosave showing 99% complete. When I reloaded, I was on the island by myself and I didn't have the quest in my journal or the unlock. I couldn't talk to the quest NPC, so I wasn't able to get the unlock.
As an aside, the NPC's faction remained friendly to me. I'm not sure if normally they would - it made the quest to rescue Melinda pretty easy.
