Conduit and Mindlash

I've done some searching, and I can't figure out why it isn't working for me.

So, I activated a pair of auras (kinetic and thermal), activated conduit, and they both went on cooldown. Great.

I have beyond the flesh up, and perhaps half the time my auto-attack from telekinetic grasp will use conduit (why doesn't it happen with every attack? I don't see any tooltips for that).

Mindlash, however, is doing the exact same amounts and types of damage it did previously. I am using a mindstone in my TK grasp.

Am I missing anything?

Edit: It just happened once. Is the damage only added some low percentage of times?

It won't apply if the normal damage was enough to kill the creature.

I've had that happen a few times when I've been running high damage characters with lots of passive damage from a weapon. If I hit something for 400 when it's only got 25 HP, doesn't make a ton of sense to run through the entire list of damage. Find something beefy to hit and you should see the results you're looking for.
And it was such a good idea...
