In the quest "Back and there again", if you give Tannen the Orb of Many Ways when he first asks for it, then it will be correctly removed from your inventory. However, if you refuse to give him the orb when he first asks for it, but you give him it when talking to him again (with the athame and diamond from Reknor), the orb will not be removed from your inventory. Then you can pick up the Orb of Many Ways that Tannen drops after killing him, so you end up with two copies of the orb.
The chat boxes with this issue are "has_material_withheld_orb" -> "orb_loan".
