[tome 1.5.10] dissapearing enterances?

Had a problem with re-accessing the dwarven city of reknor.

found it the first time around and went in the entry for a "look-see";
didn't have enough time (real world not game time) to go on so I exited back to the 'world';
saved to menu and exited/shutdown the games system;
came back later in the day and started it again and the 'entry' just wasn't there, hadn't moved either (I had a look around).

I also had a look around at some of the quests I had completed and a few others were also missing
but some extra's had also appeared.
