I ran into a problem where I can't seem to get back into Sher'Tul Fortress.
I opened it up several levels ago - killed Weirdling Beast, activated orb.
Today I took the portal to return, to complete my lich quest and it ported me to the W.B. room again. It is empty, and the door is locked so I can't proceed into the fortress.

Are you running the steam version with steam cloud saves enabled?

Yeah I chatted with someone online about that. Looks like the savegame was corrupted.
I wish there was a warning about that in-game but oh well at least I wasn't that far along.

Perhaps it would be useful to add a flag somewhere after the fortress is unlocked, so that should a regeneration of the area happen, it generates an unlocked version instead?

I just encountered this bug today, I was just using my rod of recall to head back after clearing the lost crypt (for the necro unlock). Unfortunately the game just hung at "loading level" so I had to forcibly quit it.
When I restarted it, I had to clear everything again. But this time when I tried to recall, it put me at the start of the fortress before a locked door, with lore pop-up and without any beast in sight to defeat.

This happened to me. I have a level 50 character and I was planning to use the Vault. Is there any way to regenerate/unlock/fix the room? Would it be possible to use dev tools to break in and then somehow re-verify the game?

The area did regenerate, that is the problem.
You could theoretically access it with some dev mode tinkering, but you can't re-verify the game.
