(EoR) Dominon Port Reset Issues

There are a lot of issues with the Dominion port. I'm not sure if this is new though.

Recently jumping into a Hidden Vault, I left it to come back when I was stronger. When I did eventually come back, I cleared all the Sewer Grates at that time before entering it. I entered the Hidden Vault and cleared a little bit of it but had to leave. When I came out again, the Sewer Grates had all respawned. Killing them yielded the same items as when I had looted them previously - and I now have a clone of a randart I picked up.

Then I went to Sewers 2, and that entire level was reset.....
