Okay, so it turns out there IS actually a problem with this event showing up, and it's not addon or cloud save related! Basically if the zone decided to spawn the Weird Pedestals event, and happens to put it on the third floor, then the Conclave Vault event can't be there. This is true even for Shalore characters, making them not actually have a 100% chance of triggering it.
Not sure what would be an adequate fix here. The event code seems to be working exactly as intended, but I don't know if it was intended for this interaction to occur in this case.
