Graphic Glitch with Certain Sustains

I've had a glitch for a while that certain sustains - originally just Burning Wake, but now Matter Weaving as well - don't show their graphic on the player character, instead just making a block of translucent colour.

You can see the example in this pic. Notably, the glitch doesn't affect NPCs with those sustains.
You can also see that the character's doll has vanished entirely, and only the things placed onto it - equipment and horns - remain. Unsure if this is connected.

I'm not sure if addons are at fault or not, but the problem doesn't occur until many levels into a game, so I can't easily check. Is this a known bug? Is there a solution other than 'turn graphics off'?
