Telepathy: All shouldn't appear on rares
As it says. It's too powerful.

Isn't it in most or all situations equal or worse than a ego lamp/several armor slots with up to 60 radius track, or a wand of detection?

Telepathy: All is extremely powerful as it looks behind walls, has full range all of the time, and negates blindness, stealth, invisibility, and other effects such as Sticky Smoke or Duathedlen Darkness. IMO, it's just too versatile and powerful.

Serously, I used my ~30-40(at that time) radius track lantern once when I entered Dreadfell 10 and could see every single enemy on the map including the Master. Then I easily killed everything before waking the Master.
Cooldown is irrelevant outside of combat.
The only advantage of seeing all enemies during combat is when additonal enemies arrive or when demons cast darkness all over the place, something that is usually not the case in ToME except for this version.
Blindness still lets you do everything and can be removed, stealth breaks on its own and invisible enemies have -70% damage and can be ignored.
As the droprate must be incredible low for a Telepathy:All, I didnt see one yet, it most likely has sub-par stats on the item, reducing your combat efficiency.
Its like finding an awesome voratum ring in SWL and I didnt hear anyone wanting to ban that.

It may or may not have good stats on it, it's how rares work.
I'm not sure that a radius 40 track lantern is a good example why telepathy:all isn't broken. Both of these sound broken to me.

Maybe it should just only appear on tier 5 rares.
By that point, it's powerful but has to compete with some other powerful options, and considering that it's an entirely random piece of equipment, well, I don't think it's a big worry-you may get that perfect tool and take a notable statistic hit for your Telepathy All, or you may find it on leather when you're a Bulwark.
