Right now when you complete an alchemist quest and give the monsters bits in, the quest is updated (some other alchemist elixir is also completed) and the game autosaves before giving you the reward. If the game crashes on the save (this is happening to me a lot since b42), the quest is updated but you wont get the elixir and when you try to get the reward again the quest will be updated again (another elixir will be done) and the game will autosave again, risking to a new break.
The safest aproach would be give the elixir first, then update the quest and only then save the game.
The last time I finished the bits for the Hermit elixir of the 2 class talents (forgot the name) I've got 2 breaks before completing the quest, so when I finished the next elixir my quest was done without a chance to get the final reward.
