It's possible that you and the monster have EXACTLY the same energy, so you would act on the exact same tick. Then the game kinda picks one at random to act first?

This bug doesn't happen if you just move away or do something like "wait, move, wait, move". You have to attack or use a regeneration infusion next to an enemy and then move.
Spamming Windtouched Speed since 1.5
