Seen one in Dreadfell level 1 in a vault. The same monster. I was at level 15.

A dolleg is a little OOD for the maze, but not much. A maulotaur would have annihilated you too I think and they are definitely in-depth; with a weak ego iron longsword I'm amazed you got that deep. Did you encounter a bandit lord?
If you try the maze at CL 6 you get what you pay for. I'd never do it before the Old Forest.

The minimum level for Maze (5) is 12, so you should have been getting all sorts of "shiver me timbers" level messages. Did you see those in your log?

One neat surprise in the OOD department awaited me at the beginning of the Old Forest.

Level 7 character, lvl 1 of Old Forest, escort quest. A few rooms into the zone some snakes/ants start to converge on us. "No prob" says my dwarven warrior, "i can easily defend my escortee. Here, one hack at this snake, another shield pummel at the other, the escortee will have to survive just 1 round alone with that orc warrior...".
...and the orc warrior one-hits the escortee...
...and then i miss him a few times, waiting for shield pummel to reactivate...
...and then he resists shield pummel...
...and then i have to resurrect.

It turns out it was a lvl 30 orc warrior. The second attempt went much better.

Perhaps there should be a warning when an out of depth monster is generated? ADOM has this, and it adds a nice tension to the level. Should be something like "You feel vaguely nervous."

It's been a while since I played ADOM (and it is a bit off-topic) but it's only when an OOD monster is generated AFTER the level is generated that you get the danger message.
