I guess many of these issues are hard to avoid due to the procedural generation.
1 row, left to right: Heart of the Gloom - level change, Tempest Peak - pedestal, Lake of Nur - level change.
2 row, left to right: High Peak - level change, Worldmap - near Daikara (I think), Sandworm Lair - the round part of the wall blocks the effect other walls don't.
Minimap gets clolored dots when you walk into aura. Green dots for life aura, red for spellblaze aura. If you stand on a fake aura dot all other fake dots spawned by the same real aura tile disappear.
Strange chests.

Reached the limit of 3 attachments.
Suggestion. Shell shield effect icon has a yellow background (I think it is the only effect that has a yellow background), but active sustains use yellow as well. Maybe change to another color.
