Repeated unavoidable deaths as gunslinger

I was in a vault with enemies to my left and to my right. I fired off a vital shot and immediately died. I think the vital shot spawned on top of me? I had 100% knockback and teleport resist so I'm not sure what happened, but when I tried to return, I was immediately sent back to the Eidolon (literally, not figuratively). I figured maybe two bullets were fired and I respawned back on the other one? I asked the shade to send me back and I died a THIRD time without being able to move or input any commands at all. I had a blood of life and consumed it, which thankfully did not kill me.

Relevant details I can think of was that I had the long-arm weapon equipped. I had taken the vital shot prodigy.

I had also died in an earlier vault while using the weapon, but it was just a single death and it did not cause me to die again when I respawned.

After I had died three times and then using blood of life, I could see that the shots had somehow spawned directly on top of my location (the stairs).
