Achievements discrepancy with winner character

Yesterday I first won on Nightmare difficulty, but the game did not notice that. Character in vault reports as Winner.

Winners tab does not have any winner in Nightmare.

Game does not recognize me as "won in Nightmare" and did not unlock next difficulty.

Game did not unlock Steam achievements for lvl 50 Nightmare, or for win Nightmare.
