No boss in pocket of distorted time repeatedly

So for the third time I beat embers, every single time there is no boss in the pocket of distorted time. Every time there is an error message upon entering the second floor (just reported it a few minutes ago, username Number43), and I don't get lore like on the other floors (seems to be lore 388 that's missing). This also causes me to fail to get the relevant achievement for the area.
