[1.4.8] Stuck in Slumbering Caves

I defeated the boss in the Slumbering Caves and beat the game. I did a bit more exploring in the world after I won and I decided to come back to the Slumbering Caves to go the vaults that I skipped. The problem is that I can't leave the slumbering caves/Amalthel's Prison. My Rod of Recall doesn't activate. All it says is is 'the rod emits a strange noise, glows briefly and returns to normal.'

The other strange thing is the different floors of the Slumbering Caves reset sometimes. I've been trying different floors and sometimes the floor will respawn all of the enemies and loot. It even respawns the enemies in Amalthel's Prison, but doesn't respawn the boss.

I think the problem is that part of the game isn't registering that I beat the boss, but it's also not spawning the boss after I defeated it.
