Temporal Hounds no longer functioning, Death causes lua erro

Using steam, Win7 x64

This happens on any cast of the spell, automated or manual. The spell won't actually turn off, but one hound will remain active. Only one though, it will never summon hound 2 or 3.

This second one happens on death: Any death, and after it leaves a blank screen as if you continued playing but no longer have a character'
