[1.2.5] Item graphics changed after using Item Vault

Sometimes when I send something through the item vault, they have a colored character like "]" or "(" instead of the normal graphic:

If I equip the item, it doesn't show up on my avatar.

The character I retrieved it with has used the vault a lot, so I'm not sure if that's a factor. I transfer a lot of stuff to this character for storage, and have many items on the floor in my fortress.

Looks like its a problem for DLC items.

That could be. I never had the issue before getting Ashes DLC.

After using the vault some more, I'm only seeing this happen with Ashes uniques. Specifically, the Jaw of Rogroth had the same issue again.
