As you can see, the only two deaths were self-inflicted! I now have two copies of myself standing around in the fortress, the first of which was generated at the same time as my death(s) and glorious rebirth. When I move into one of them, a new duplicate is created and I am moved to the teleporter (but I suspect that's a separate issue!)
In case it helps, I killed myself by disabling Blurred Mortality and repeatedly zapping myself with my staff's projection ability, just to ensure that I didn't overdo the damage and cause a problem (guess that didn't work out too well!)
