The Ruined Dungeon Bug

I was in the ruined dungeon on the normal campaign and I only found 5 / 6 bosses. After digging out the entire map in search of the sixth boss, I found a green spell being cast on the wall of the vault, and since everywhere else in the dungeon was entirely clear, the only place the last boss could possibly be was inside the vault, making this dungeon impossible to complete.

I doubt it's a common issue as no one else seemed to have had it happen to them, but nonetheless, I thought I'd drop a line on here and let you know.

Had the same thing happen to me today as well. I cant say for sure if I killed 5 or 6 guardians but I had 6 pink item in the transmog chest... Dug out every single wall in the map, spammed AOEs, even walked every tile... still have one inactive orb, and no signs of life from any invisible/burrowing enemies. Really disappointed because I want to unlock the Infinite Dungeon, but after doing a little research it seems this bug has been around for a while. Guess I'll have to get it on the next character.
