Zero accuracy in High Peak

So as I was making my way through HP, I got to around the sixth floor when suddenly my character wasn't able to hit anything. I look at my debuffs, the only thing I've got is the zone debuff, which didn't reflect an accuracy penalty in the tooltip. My character sheet says I've got 0 accuracy, and when I use Perfect Strike (for the +100 accuracy) it says I've got 36. This effect persists all the way to the 10th floor. I've looked all through my character sheet and my inventory to see if there's a reason I should have 0 accuracy, but I can't find anything. Am I missing something, or am I just bugged?

I have the exact same issue, also on a Berserker. http://te4.org/characters/25065/tome/4d ... e64371d3b9

Mine was caused by fighting a Farplane boss Oozing Horror, which lowered my accuracy permanently somehow, even to well below 0.

Also having this issue, on my Berserker. Happened first in High Peak on the level with the Aura of Light which also gives -10% accuracy. Was fighting the stair guardian which inflicted a crazy number of physical debuffs. After porting away and resting I noticed the accuracy drop to 0. Only the sustain Berserk increases it to about 20:
