What exactly happened when this appeared ?
[tome] joylove: You can't just release an expansion like one would release a Kraken XD

Had this happen yesterday. I was level 8, and was returning to the Mire to fight Bill...
Moving around was VERY slow(I wonder if this is related to the bug). When I reach the stairs to level 2, guess who's there?

Image

My own character, except it is from when I was level 1 - including equipment, sustains..so definetely a duplicate from when I first was there.
At first, I actually considered this could be a Chronomancy easter egg, but I'm a Mindslayer so...definetely a weird bug.

As it's on the stairs, it fits with rainbowjose report that it happens when entering a new level too...

I had this happen too, but I didn't get any screens, sorry.

I recalled out of the snowy forest dungeon near the elf city. I can't remember the name. I was a necromancer with a few minions. When I got to town I noticed that I didn't have any in tow. I had a few slots open, so I summoned more and only got 2. The other 3 were nowhere to be found. When I went back to where I recalled my clone was there along with my 3 minions. I ported the clone away to hopefully kill the uncontrolled minions, but they just ran to him without taking damage. I then killed the clone and the minions disappeared. It still showed that I had minions under my control though they were nowhere to be found. I removed my necro aura to try to kill them off, but only the 2 summoned died. I then killed myself and they appeared in the void, or whatever that's called. They died off without the aura and I was able to summon my full amount again.

I also got game slowdown on the level with the clone.

Edit: I am a halfling necro.

I had this happen to me as well; playing as a yeek necromancer.

Went up from Trollmire(3) and my 3 minions did not follow, but were still showing in the status pane. I summoned and only got two more minions.
I then went back down and there was a duplicate of me with his three minions. I killed the minions to no avail.

I killed the duplicate and leveled up :) This allowed me to unlock shadow tunnel. Using it brought the 3 minions back on screen. I could repeatedly kill them, with the killed message flying, but got no experience. Bumping them turned them into a duplicate of me. These duplicates were different; spells cast on them hurt me instead of them. They also did not cause the "2 out of three inputs ignored" state.
