In specific situations, monsters can be damaged by splash/blast traps (or possibly other AoE effects or spells) without alerting if they are not able to see or sense the player. Not sure exactly how to deal with this (having them walk into the trap again doesn't seem like good AI) but maybe there's a way to have a monster 'explore/search/investigate' for a couple of steps when it takes damage from an unknown/unseen source?
