[b31] Missing log entries ?

Often when I take a huge spike of damage, or die unexpectedly, I check the log to see what caused it, and frequently it seems to be missing from the log.

For a recent example, my last character's death log was :

Scrofulus uses Re-assemble.
Urkis, the High Tempest casts Ice Shards.
Urkis, the High Tempest speeds up.
Urkis, the High Tempest hits Scrofulus for 11 lightning damage.
Scrofulus forces the iceblock to shatter.
Scrofulus is free from the ice.
Scrofulus hits Scrofulus for 5 light damage.
Scrofulus hits Iceblock for 5 light damage.
Scrofulus hits Iceblock for 5 physical damage.
Scrofulus hits Iceblock for 30 physical damage.
Scrofulus hits Iceblock for 4 lightning damage.
Urkis, the High Tempest casts Lightning.
Urkis, the High Tempest hits Scrofulus for 107 lightning damage.
Urkis, the High Tempest killed Scrofulus!


After re-assemble, my hp was 400 or so - afterwards I see one hit for 11 and one hit for 107 from Urkis, and one hit for 5 from me (that's another bug with iceblocks but I have seen that one reported previously ), for a total of 123 health, but was, apparently, killed by the 107 damage hit.

It usually seems to happen with mages - with my temporal warden I checked a few times on the log when I got dropped to 10% health by a mage, and it would only list one nuke for 100 or so.

Was the enemy that caused it outside of your line of sight? I know that's one (legitimate) way to have missing log entries.
