If you have Probability Travel up while you're inside a no-teleport zone (like a vault), then walking into a normal door will cause nothing to happen rather than trying to open it -- I assume it tries to teleport through the door instead of opening it, fails, then gives up.
Happened to me in this vault. Turning Probability Travel off fixes the problem, of course, but it seems unintuitive, so I assume it's a bug.

This has always been the case and I am still not convinced that this is an issue.

You also can't activate golem quest with pt on. If you then turn it off, it still doesn't activate. That is, walking into golem statue. This is with shadow blade with pt.
