I failed all escort missions but the achievement didn't pop up, are there any other requirements for this?

For that achievement you need to have all of your escorts die, but not kill them yourself. Need to actually accept the quest and then have them get killed by enemies.

For the Backstabbing Traitor achievement you need to actually murder six of them yourself, so these two achievements are mutually exclusive in a run.
