When I use Inflame on myself, I got the Blood Heat triggered.
When I use Inflame on enemy (and it hits), I DO NOT get the Blood Heat triggered.
P.S.: AND!!! There is nothing about "intentional" in description of Blood Heat. It says, it triggers every time a Fire or Bleed effect is applyed to me or by me. And the DoT, procced when hitting a target with Inflame on it, surely MUST BE a Fire effect.

Another bug is related to Nature Combat...
1. Nature Combat talents are counted against those 4 Class Talents that you can respec while in safe zone.
For example: I reach 3rd level. I put 4 class points into Flamespit and save changes. Then, I put 2 Generic Points into Elusiveness and save changes. Now, I changed my mind and want to take 4 points from Flamespit and rearrange them, but... I can take only 2 points away!
2. After Class Points respec, I noticed that the "First Discipline" changed from one talent to another. That is really frustrating!
1. Nature Combat talents are counted against those 4 Class Talents that you can respec while in safe zone.

I just investigated this myself and have confirmed that it is working completely fine. However, I discovered another bug while checking this (unrelated). A good thing I looked, it seems!
Also, you're right. It doesn't say "Intentional" in the description because this was actually something I'm changing for the next patch, and I completely forgot, heh...

That's a T-Engine thing; I can't really fix that. You should ask the engine's actual developers about fixing it.

Strange... That's a very old bug that I fixed when I made "Natural Combat" talents irreversible. Did you take out the points at the last second? You might have misunderstood something.

I took out as many of my addons as possible to give ritch a shot and see if intentional effects were working. Addon folder ended up looking like this:

Now, Inflame was working fine, and so was Open Veins! So I can say gladly that Celestial oddities isn't messing with them. What DIDN'T seem to be working was Blood Heat. Now, checking, you just said that Inflame isn't meant to proc it. Which seems odd, I'm not sure how they are meant to proc it, but sure, as long as it's deliberate. Open Veins wasn't proccing it either. Only thin I saw proc it was when an enemy made ME bleed...
So, might be a bug, might be an addon issue, might be working as intended.

Flutter at talent level 5.2 says it is giving me 123 defense, but the character sheet is showing it only going up 45.

Defense, like most stats, is affected by diminishing returns. I believe it is every 20 effective defense you have, it takes one more point of base defense to give you one point of effective defense. So, 20 base = 20 eff, 40 base = 30 eff, 60 base = ~36.67 eff, 80 base = ~41.67 eff, 100 base = ~45.67 eff, 120 base = 49 eff. The fact that you are getting 45 effective from a boost of 123 means you have something like 16 defense from other sources (dexterity, hat, cloak, etc).

A friend of mine is playing this addon and wants to report this bug (I think playing as fire drake):

Inflame isn't meant to proc it? It does for me. Just the 'Inflame' effect itself, in fact, not to mention the burning damage caused when an 'Inflamed' target is hit by fire.
The bleeding from Open Veins procs it, as well.
Probably still an add on conflict. Hate to say it, but Celestial Oddities looks like the most likely culprit out of your selection of add ons.
(For reference, the only two non-DG, non-Neka add ons that I use while playing OoTS are Inventory Sort Order and Faster Run Rest Explore.)

'I just noticed that for the past 10,000+ turns, Eelkas (one of his dragonligns) still has a Fed Upon effect, negative a lot now, from a named ooze. This was originally gotten in the Sludgenest. I am not sure of impact now, but maybe it's causing [non-fatal] LUA errors when I exit a dungeon. Erm. How to clear?'
'Uh oh! The dragonling is causing a LUA error, related to strategies or something. And now ambushes are inescapable in the game -- can't load the overland map afterwards. Hrm... Maybe I could recall out of them (not stuck right now, saved first!).'
Is this fixable? Any recommended workarounds?

"Diminishing Returns", as nsrr said. It's a bit misleading, I know, but you'll be glad that's how it works (since some enemies could have like 140 Defense even AFTER diminishing returns).

Indeed! Some Drakelings grow quickly, while other ones grow slowly. This is directly affected by whether the Drakeling has learned "Growing Bones" or not (each Drakeling has different Natural Combat preferences, so some may tend to be very small while others are very big). This is mostly relevant for if you're a Fire Drake learning "Lumbering Beast" and have "Relativity", because it can decide whether you're able to see them or not, heh...

The most important thing for helping me fix bugs is a Screenshot of the LUA error. I can usually instantly identify the problem if I can see exactly what the error is.

"Fed Upon" is sticking around because it's programmed only to be removed when the source of the effect is dead (I'm supposing he left the zone without killing the enemy that did it). It's mostly harmless (since "Fed Upon" is a helper effect that tells the actor which target to give the bonuses to)... Mostly a strange interaction (since ToME is not exactly designed to support "permanent" party members).
If you're in "Developer Mode", you can fix it by pasting this into the console:

Located an add-on conflict in "Celestial Oddities" ~ "mod.class.Actor.on_set_temporary_effect" is NOT superloading!
This is interfering with absolutely EVERY "on-status" effect in Odyssey of The Summoner ("Blood Heat", the entire "Ego/Mantis" tree, etc...). This will also conflict with every other add-on that is attempting to use that function (which I can guess is A LOT, to be honest...)
