In terms of difficulty, the beginning (first 15 to 20 levels) is much easier than the normal game. The mid-game is of similar difficulty and the prides onward are more difficult.

The final battle was much more difficult than insane for a powerful adventurer build, but not as bad as madness.

Two things that have amused me so far about playing this mod:
1) Just how lucrative the lumberjack village was.
2) Getting a prodigy point long before finishing the T1s.

Ok, two more things that have amused me playing with this addon:
3) Rocketing up in levels and then hitting 34 and getting absolutely zero experience because 26 is the highest level enemies spawn as in the T1s.
4) Creating characters with a life rating of 0.
