I took the Dark Crypt at 25, right after getting Lich. I was clearing out uniques and bosses, no problem, and then... a rare brawler hit me out of nowhere when I thought the area was clear.
Le sigh... I shouldn't have gone into the crypt, though I almost always do (fifty-fifty chance of making it through).
Apparently, with the extensions and additions, Insane is much, much harder than it used to be.
TAKE 2:
I had a level 50 Necro, completely indestructible... until the Vor Pride boss who absolutely demolished me. Just massive damage that took me down so quickly, I was shocked, seeing as how nothing had been able to hurt me for 25 levels or so. I could have taken another stab at it but I don't mind letting characters die, even really promising ones.
The lesson is that... with Embers and Cults and Ashes, the game is quite a bit more difficult than the base. It demands much more careful play.

Currently around level 20 and am busy desperately trying to run from Urkis with a Mirror Image rune... so yeah, that's fun.
EDIT: Died at 22 against Celia. Took 1800 damage in one turn so... yeah, not living through that.
Back luck in my recent runs. I can take down Urkis and Celia before level 25 about 75% of the time but not this time, ha ha. Ouch.
Currently around level 20 and am busy desperately trying to run from Urkis with a Mirror Image rune... so yeah, that's fun.

I now have a level 45 Necro who just picked up the Writhing Ring of the Hunter, which grants a third prodigy. So what to take? I've gone Lichform and picked up Adept but now what? I managed to traverse space so I can pick up Aether Permeation, but I'm also considering Master of Disaster (additional spellpower and status effects), Eye of the Tiger (cooldowns on crit), and even Cauterize or Ethereal Form.
Cooldowns are my biggest problem right now but I know that the last run of the game can be brutal and I've already lost a level 50 Necro in the East. Any thoughts?
EDIT: Still haven't decided on what additional prodigy to get but my damn shadows triggered all four of the terrors in a Gorbat Pride vault. All four are just sitting by the door, waiting for me, so... no thank you. Two prides already down and this feels like a winner. I've tackled the boss most likely to kill me, which is the Vor pride boss, so I should be all clear from here.

Try Blighted Summoning? If you have any sort of summoning available. Might be useful to turn your minions into a more effective meat - er, bone? - shield.
Also Windtouched Speed to give you 10% CD reduction, but I don't think you'd have the required prerequisite for that one.
Not sure if Cauterize would be useful for a Lich, since it will only activate when "you receive a blow that would kill you" (direct quote from the description), unless it will also activate when you passed the 0 hp threshold?

I actually ended up not using the ring at all. The High Peak had one tricky fight but otherwise went smoothly and the last battle was standard... Putrescence and Corpse Explosion and just unloading on them. Didn't bother with the portals, kept Aeryn alive, and took down Argoniel first even though I was trying for Elandar. He went down soon after.
