There was not an enemy on sight... these kinds of death really piss me off.
Make sure you have radius 10 sight (light radius + heightened senses = 10) before you do SWL, or other dark dungeons. If you go in with less than that, expect getting hit from out of sight often!
The drakes and wyrms have huge range for their breath, all the way across the screen. Have physical ward or torque that reduces physical going. Otherwise be able to heal/shield immediately because as you saw getting hit with multiple breaths at the same time is a very real problem.
This is also where track/earths eyes/ arcane eye/vision rune or wand all can come in handy. See them ahead of time and take counter measures. The tools to prevent this are available

The latest paradox made it to level 29, but was abandoned due to The Master being in the room with the stair down. No real way to hit and heal when the dude is in the room you enter the level on.
Always fun to play... always manages to sneak up and whack my squishy characters.
