Char in Progress: Dying alot at the Summit - advice?

After dying 4 times in quick succession to the boss fight (Nightmare/Adventure), with only a Blood of Life left for revival, I'd like some advice / help. My character's a Shalore Temporal Warden, and I left out the inventory in the first post (inventory's in a pastebin link at the bottom of the post, if it matters).

I can't seem to be able to get to safety for long enough to recover timeless and/or invigorate.

Does anyone have any advice or character build comments? My goal was to keep up the ~70% resist-all and to mostly do damage with Warden's Call and Arrow Echoes while running. Turns out that blinking around constantly isn't the safest thing to do... Maybe I should replace the regen/wild infusion with controlled phase door?
