Fylish Rewards?
is that an item, lore or new class,race?

Just looking at the lua error in bugs it looks like allowing the race orc. Though someone who's done it will know for sure. Is this going to be permanently avaliable? Do I need to unlock it while I have the chance?

I cracked open the savefile, when it decided to not give me whatever the reward was the second time.
From what I saw, it allows you to load a prebuilt one time for an Orc race character that will always be named Kormak(I think it was) and will always be Male. You can pick from the Warrior/Mage/Wilder/Defiler types for the Orc.
IIRC the stats were like +4 Str, +4/+1 to...something else, -2 Magic, -2 something else, 130% EXP mod and Orc racials(same as the ones they have fighting you, pretty good stuff).
Pretty strong race, but you only get one crack per run of Fylish, if I read right. Not really a massive deal.

What's up with this quest? I got it only once, stupidly died in there, and now I can't seem to get it anymore?

It's bugged and ends your character if you complete it. Just don't bother with it...
