Some combinations of class, base monster, and talents can be quite game breaking. I'm not talking about simply powerful combinations, like a Radiant Horror/Sun Paladin/Paradox Mage with tons of healing and Second Life for recovery as well as high damage, sometimes the RNG hates you, and that's a part of the game. This is about unforeseen interactions like these:
(Note: I haven't actually seen all of these combos, some may already be forbidden/fixed)
Disruption Shield + daelach, champion of Urh'Rok, forge-giant, or blood lich: 100 mana regen (+1000 extra mana for the daelach) means that they recover from 200 damage each turn passively (At max power Disruption shield), or about 2000 damage if they pop a manasurge rune. After you beat down the shield, it will explode for a huge amount of damage. Not every build has access to a sustain removing talent, making these enemies practically invulnerable.
Death Blow + champion of Urh'Rok or forge-giant: 100 stamina regen means its bonus damage from draining stamina will always be fully powered, and the stamina loss is meaningless. This is somewhat countered by the fact that it has a long cooldown.
Dimensional step + boss ESP: bosses always know where you are, which usually means that they chase you down when you escape. However, if they have a powerful enough dimensional step they can teleport right next to you immediately after you use a teleport rune. I've seen this at ranges of 20 or more.
Brittle clear oozes: they split, even as bosses. I've killed 50+ of one stair boss in the ID, it was quite tedious.
Quicken Spells, Metaflow + non archmage spells: may be intended, but I believe that they work on all "spells", (eg. everything affected by silence), not just those under the spell category for talents.
Speed stacking: monsters can stack many more sources of global speed than the PC. The worst I've seen is Essence of Speed + Haste + Borrowed time, though it could easily have also had Speed, an intrinsic bonus, and maybe elemental harmony as well.
Time Skip: At high levels, it can be chain-applied, especially if the enemy is faster than you. With a high enough level in spacetime mastery, the boss can overcome continuum destabilization many times in a row and kill you without giving you any chance to react.
