Logs are a bit weird sometimes, but this is almost certainly Catalepsy. The Ogric Masses know Rotting Disease, which lasts a ridiculous number of turns on NPCs. Being Nightmare and level 38, they almost certainly have it near the max duration which seems to be 25 or more. If you were taking 28 damage a turn from it, that's 700 damage - Catalepsy would probably be a 150% hit, so yeah, 1k+.
I really think the super long diseases should be toned down a little, 99% of the time their damage is so low it's just an irritating way to stop you resting after killing a ghoul, but sometimes this happens. Probably better to increase the damage per turn and put the duration down to something more sensible rather than 20+ turns.

It was indeed most probably catalepsy, however it happened so much earlier that I didn't consider it relevant. The duration is just silly. The modifier was 124%.
Obviously I didn't notice the exceedingly long duration of the disease, 30+ turns. The character at the moment would have survived up to 600 damage, but not 1000+. I think either the damage per turn or the duration should be lower, or the catalepsy modifier put well under 100%. 1k+ damage in one action, by a normal ranked mob, is just not fair.

the duration of a disease is supposed to be 6 turns.
tome tips doesn't show any scaling, so unless the higher difficulties actually cheats, that should not be possible.
