Just found a vault in the Master's Tower (Level 20 or so Wyrmic) that I wasn't able to map out properly because it had a whole bunch of level 42-44 undead of various sorts including a heavy mix of elites. Was able to kill 4 or 5 but they just kept coming and the risk just wasn't worth it, so I split. Whatever kind of vault that was (was definitely a vault, gave me the confirmation when I opened the door), it was nasty.
In terms of sheer likelihood of killing a character who decides to crack it open, I'd say the "worst vault" is a toss-up between the ones with 9 leveled rattlesnakes (I've only seen then in Trollshaws), and the little 4-square ones full of scrolls that have leveled skeleton mages guarding them in front ("La la la, I'm a level 9 archer wandering down a side path in the Old Forest, killing the little ants that I see. Hmm, what's that red... AAARRGGGGHHH, IT BURNS!" BLAMMO!)

I just did that vault in the master's tower with an alch and I found it to be quite easy. Especially with 5 in stone touch but I probably would have been able to do it without. The loot was really great and I ended up with a lot of ego scrolls, ego potions, and some really sweet gems 8D. It was totally worth the risk. The Master on the other hand...
To be honest I have never been disappointed with vault loot, but maybe that is just because they are pretty easy with an alchemist (the only class i've tried since beta 4 heh)

I think it depends on the kind of vault. For instance, one vault I've encountered a few times involves a skeleton mage standing outside of a room containing four random scrolls. While I appreciate the imaginative idea of raiding a skeleton mage's tower, the loot is a let-down. Granted, killing the mage wasn't so hard either. Similar with one that involves a big castle-looking thing with guard trolls and interior trolls. Easy fight, not so interesting loot. Neat concept.
On the other hand, honey tree vaults always seems to contain at least one piece of galvorn (even in the Trollshaws,) and once in Tol Falas I ran through this twisty-turny thing that was just a series of fantastic items and big monsters (reminiscent of TOME2.)
So, not all vaults are created equal. I don't particularly mind the weaker vaults, now that I understand that they're not so heavy on the stuff anyway. It's just annoying when you expect some big "Ta Da" item in a vault that only consists of a skeleton mage.

Those vaults are tons of fun, especially in beta11 with drakes summoning drakes.
