The item curse of nightmares level 4 effect can be crazy

I'm seeing stuff like this on a regular basis from the level 4 item curse thing curse of nightmares, sometimes even worse than pictured here so I can't see where anyone is through all the purple:

It's really any enemies that are any threat will promptly give me dozens of summoned terrors. None of the other item curse effects are anywhere near this significant.
