I used Quantum Tunnelling (the 2nd skill in the CoE Rift tree) and targeted a rift about 9-10 tiles away; got a Lua error. I have the dúathedlen heart lamp equipped, if that makes a difference. This reproduces consistently, so hopefully it's not hard to fix. Make a Reaver, give it Bone Shield and sustain it, exit to the world map when you have less-than-max bone shield charges.
