I got it several times on the first boss... You could spam kill him and that steam giant boss with new characters, perhaps? Otherwise, I'm not sure, unless you believe in ritualistic dancing for the RNG God.

I've had this happen to me as well. I'm not sure exactly when, but I think it was around when I had a chain of paradox anomalies (Overclocked Radius's effect) while attacking that Solipsist boss in that "Way into old ruins in the Eruan desert". I charged/one shot him though, so I don't thi...
