I think I have found an explanation for this. The Iron Acorn spawns in the scene "A Haunting Dream" (lore #267). After been teleported in, the character has to walk north along the path to pick the acorn up, before he kills all the NPCs there. When the character is teleported into the scen...

The acorn is missing for me in 1.3.3.

In case it matters:
The path I choose is "kill him, keep book, I need your help."

Are you trying to implement some mechanism along the line of Skyrim SexLab addon such as this one?

You will get a .teaac file, which is a zip file.
It consists of .lua, .png and .ogg files.

I disabled all add-ons by removing them from the folder "addons", but the problem remains.
