Also, as great as armor is on lower difficulties, on madness sometimes it really loses its luster. Enemies regularly have base weapon damage in the several hundreds in madness, and I saw a high peak warhound get to over 2k. That link is to my madness TW winner, that I just won yesterday. It's up to you whether or not you want to believe it, I did vault over mercy so that doesn't count under the "S" rules listed, but it doesn't really matter that much. Having it in B tier is totally wrong though.

The AI changes is actually the thing that worries me the most. Enemies can get a lot stronger and more numerous than the player on higher difficulties, so if they act intelligently that can make it hard to overcome that. Seems really bad for melee classes, since usually there are a limited number of...

Oh, and darkgod gave out invulnerability when I was in the final fight, too, when Elandar was in middle health range. If there ever was a time to get invulnerability, its hard to choose a better one.

Keep in mind at least some of the "lose x% of a turn" procs are handled as turn_procs on the target, at least the one I checked in the scoundrel tree worked that way. If it is handled that way, that means that proc will only work once until the target gets another turn, not the source appl...
