I'm pretty sure it does proc, as I'm running a test adventurer and the blight damage is always multiplied by the proc multipler. I'd rather not see each part as a guaranteed drop, but Imd definitely appreciate having a chance at it. Even if it were 1 guaranteed, 2 at 50% or something like that.

I've most recently found Wheel of Fate also in a demon statue (also in Searing Halls 2). But I've seen it drop from monsters in other zones in previous versions. So unless it's been changed, I'm going with no.
