Do you happen to have multiple copies of the DLC? I had that and was getting the same results. If so, just delete one of the copies and you should be good.

I think you get it when you actually leave the demon island. If you've already done that and not gotten the achievement/unlock, then yes, I'd say it is a bug.

That's a bug. I went through with a doombringer and saved her just fine.

I'm still having this problem, and I disabled every single addon except for official ones (ashes, item vault, etc.)

I do seem to have ashes listed twice though for some reason, so I'm guessing there is some conflict there.
