Just to chime in, this also happened to me while playing an Elven Reaver. When it occurred, I was in Exploration mode and had just been killed. First, I had my Chain Lightning spell disappear. Then I lose the Flame spell. They just disappear from the action bar at the bottom and I no long have a way to access them.

Any thoughts?
