The same situation happened to me while playing Dwarf Corruptor. I got killed by one of snow giant uniques in Daikara, then immediately died from blight damage while on Eidolon Plane. Asking Eidolon to return me to the place of death teleported me back to Eidolon plane instead. After that, yeah, all...

I ran into exactly the same problem today as described by Robsoie, though I unfortunately don't have backup saves.

EDIT:However, turning OpenGL shaders on solved the problem.
