1) Ghoul retch fails to mention that it scales with constitution.

2) Ghoulish leap fails to mention that the leap is followed by an attack.

3) The flameshock spell does not mention that its damage is a dot.

Edit: My my, that's one ugly-ass portal, but I found it at last. Still, I'd suggest looking up into why auto-explore ignores it, and/or maybe changing its skin? But that's just my 2 cents; now that I found it, it won't bother me anymore.

Is your character supposed to notice it when auto-exploring? I swear I looked around the level and found nothing.
