Update: Discord user Astralinferno found the relevant code. Apparently, all shadows stop using fade after they hit 100% damage reduction from their master. This typically occurs at talent level 5+.

I'm also not 100% certain of this, but I didn't notice the issue at all until after I killed myself to turn into a Lich, and it's very possible that the problem started there. I'm not sure how or why not-quite-dying would affect the Fade talent, but it is a potential starting point for investigation.
